Who funds health and medical programs?
It depends on which of three things you are: a researcher, a care provider, or a community health program. Researchers look to NIH and disease-focused foundations. Providers, especially clinics and rural facilities, look to HRSA and state health departments. Community health programs draw on a large population of health legacy foundations, created when nonprofit hospitals and health plans converted or were sold, whose entire purpose is funding health in a specific region.

First decide which door is yours
Health funding punishes an unfocused search more than any other vertical, because the word covers three separate funding systems. A research grant from NIH, a service delivery grant from HRSA, and a program grant from a regional health foundation have different applicants, different review cultures, and different definitions of success. Aim at the wrong system and a strong proposal reads as a category error.
Once the door is right, the search narrows fast. NIH and HRSA opportunities are published with detailed eligibility. Foundation health funding is where the reading gets harder, and where the filings help most.
The funders to know
- Health legacy and conversion foundations
- When a nonprofit hospital or health plan converts to for-profit, the proceeds typically endow a foundation required to fund health in the community it served. These funders are place-bound, health-mandated, and chronically under-applied-to. If one covers your geography, it belongs at the top of your list.
- Disease and condition foundations
- Foundations organized around a condition fund both research and patient services. Their giving history, visible in their filings, shows which side they actually favor, which is frequently not what their website emphasizes.
- HRSA and state health departments
- Service delivery money, from community health centers to rural health programs, moves through HRSA and through states. These are published opportunities with real deadlines, and expired listings are flagged in Quillify rather than left to waste your afternoon.
- National health philanthropies
- The large national health funders are real but heavily oversubscribed, and several fund only by invitation. Check the accepts-applications signal on a funder's profile before spending a week on one.
